SCHOLARSHIP
TERMS AND CONDITIONS
Scholarship Applicants
All applicants are required to review and agree to the scholarship terms and conditions before submitting an application.
1. Scholarship Use and Restrictions

Scholarship funds must be used only for the program, camp, or training listed in the student’s application.

Funds may not be transferred to another student, program, or activity without written approval from Youth Play USA.

If the student does not attend the approved program, funds must be returned to Youth Play USA.
2. Vetted Program Requirement
-
The camp, organization, coach, or instructor must be a vetted and approved provider.

Youth Play USA reserves the right to deny funding to any program that does not meet safety, quality, or eligibility standards.
3. Verification and Documentation
-
Students and parents agree to provide any additional documents needed to verify enrollment, financial need, or program acceptance.

Youth Play USA may contact the school, coach, or organization to confirm participation.
4. Attendance and Participation
-
Students must attend the program in full and follow all rules set by the organization.

Failure to participate may affect eligibility for future scholarships.
5. Code of Conduct
-
Students must demonstrate respectful behavior, good sportsmanship, and positive representation of Youth Play USA.

Serious disciplinary issues at school or during the program may result in loss of scholarship eligibility.
6. Media and Photo Permission
-
Youth Play USA may use non‑identifying photos or stories for promotional or reporting purposes.

Families may opt out of media use without affecting eligibility.
7. Reporting Requirements
-
Students may be asked to complete a short reflection, thank‑you note, or follow‑up survey after the program.

These reflections help Youth Play USA continue offering scholarships.
8. Financial Responsibility
-
Families are responsible for any costs not covered by the scholarship, such as transportation, equipment, meals, or optional add‑ons.
9. Accuracy of Information
-
All information provided in the application must be truthful and complete.

Providing false or misleading information may result in disqualification.
10. Eligibility for Future Scholarships
-
Students who complete the program and follow all requirements remain eligible for future opportunities.
-
Students who misuse funds or fail to participate may be restricted from reapplying.
11. Organizational Rights
-
Youth Play USA reserves the right to modify, pause, or cancel the scholarship program at any time.

Submission of an application does not guarantee funding.
12. Agreement
By submitting an application, the student and parent/guardian acknowledge that they have read, understood, and agree to these Terms and Conditions.
